What is the Collaborative?

Many adults in the U.S. are suffering from undetected or uncontrolled peripheral artery disease, PAD. Awareness of the risks of PAD, including reduced quality of life, amputation, heart disease, stroke and death, is low.

The PAD Collaborative unites organizations and experts committed to advancing the PAD National Action Plan with the goal to increase public awareness and understanding of PAD, prevent complications, address health barriers and improve quality of life for people living with the disease.

Peripheral Artery Disease (PAD) Lower Extremity Amputations Heat Map

The purpose of the map is to spread awareness and assist advocacy efforts to reduce lower extremity amputations. To create the data for this map, the total number of people diagnosed with PAD in each state, congressional district and county were followed over a five-year period to determine how many underwent lower extremity amputation.

In addition to geographic data, this map includes filters to explore demographic characteristics such as age, race/ethnicity and sex. This information can help better show how PAD affects different people and how this challenging problem may need different solutions.

Map description:The interactive map above illustrates lower limb amputations per 1,000 Medicare patients with PAD. The map utilizes the 117th Congress for congressional data. Darker colors indicate areas with higher amputation rates, and lighter colors indicate areas with lower amputation rates. Gray represents areas where the data is either missing or suppressed to protect patient privacy.

Data Source: Medicare and United States Census Bureau data were used to create this heat map. Amputation rate is defined as the number of lower limb amputations per 1,000 Medicare patients with PAD.

Meet the Collaborative:

The long-term success of the PAD National Action Plan depends on the collaboration and coordination of many groups and health care champions committed to conquering this disease. More than a dozen organizations and hundreds of volunteers have already raised their hand to work together to advance the National Action Plan.

Leadership

Dr. Marc Bonaca serves as the current chair of the Collaborative. Dr. Bonaca, together with a Steering Group comprised of representatives from seventeen organizations are responsible for guiding the decision-making process and inspiring action of Collaborative members ensuring National Action Plan implementation is moving forward.

Six Goal Committees, each led by volunteer committee cochairs actively work on implementation strategies outlined in the National Action Plan.

PAD Collaborative membership is open to both organizational and individual participation. Members agree to work together to have a coordinated approach to implementing the action items and tactics identified in the National Action Plan.

The PAD Collaborative is coordinated by the American Heart Association.

Public Awareness:

Professional Education:

Enhance professional education for multidisciplinary health care professionals who care for people with PAD.

Detection and Treatment:

Activate health care systems to provide enhanced programs for the detection and treatment of PAD patients, with a focus on understanding and addressing patient-centered outcomes.

Public Health:

Reduce the rates of nontraumatic lower extremity amputations related to PAD through public outcome reporting and public health interventions.
